W2 doesn't find phase space channels for some processes with Z,W,H in the final state

For some processes (see attached files for a list) W2 fails at integration with an error

| Generating phase space, writing file 'eeZ4.phs' (this may take a while)
*** Error: Process 'eeZ4': no valid phase-space channels found
*** Error: Process 'eeZ4': phase space setup failed, skipped

This happens for some of the processes e1 , E1 or u,U -> some Higgs,Z,W-Bosons

 process eeZ4 = e1, E1 -> Z, Z, Z, Z  ! in the above case

Some others work. All those processes should be possible at tree level.

Tested with the recent r1606 (So maybe WK is already working on this.)

Re-implemented the loop over the number of allowed off-shell lines, as it was done in W1. Checked with the attached bug9_ee.sin (phase space is generated for all processes).

Still, for finer control the expert user may access parameters like phs_t_channel or threshold values.
